*Note: Our office does not provide documents or forms for filing. If you need these documents, please visit:

If you represent yourself, your forms must go through a review. A reference attorney at the Law Library’s Walk-In Case Review will perform this review. Visit the Self-Help Center for more information.

For more information regarding name changes in Texas, please see chapter 45 of the Texas Family Code.

You must pay the filing fee at the time of filing your petition with the District Clerk’s office. Please see our Fees section for more information. If you cannot afford these fees, you may submit a Statement of Inability to Afford Court Cost along with your petition.

When submitting your final order, complete the VS-165 form.
